Summary

The facility failed to ensure medications were available and administered as ordered for two residents reviewed for medication availability. For one resident with hereditary spastic paraplegia, physician orders included diazepam 0.5 mg twice daily for anxiety and metformin 500 mg twice daily for diabetes. The January 2026 MAR showed diazepam was documented as not administered on two doses on one day and one dose on the following day, and metformin was documented as not administered on one morning dose. There were no corresponding progress notes explaining why these medications were not given, and the resident’s MDS showed a BIMS score of 15 out of 15, indicating the resident was cognitively intact. The pharmacy review and staff interviews showed the diazepam was not available through the Pyxis inventory report, and the pharmacist stated the pharmacy never received the physician order for diazepam. The pharmacist also stated the metformin had been ordered and sent back to the facility on the same date it was ordered. An LPN/supervisor confirmed the diazepam was not ordered and stated the process was to ask the hospital to order medication if the hospital forgot to send the prescription. The facility’s medication administration policy stated medications are to be ordered, prepared, administered, documented, stored, monitored, and disposed of safely in accordance with practitioner orders. For a second resident with mycobacterium avium complex, the physician ordered Arikayce Inhalation Suspension 590 mg daily for pulmonary infection. The MAR showed multiple missed administrations across May and June 2026. An LPN stated the resident returned from an infectious disease appointment with the Arikayce orders and that the family member was contacted to bring the medication into the facility. The LPN stated she was unfamiliar with Arikayce and how to administer it, and that respiratory therapy was to administer it on weekdays while floor nurses were to administer it on other days. The resident had to show the LPN how to set up the Lamira nebulizer system, and staff interviews confirmed there was no training for nursing staff on Arikayce administration.
