Incomplete Water Management and Infection Surveillance

Summary

The facility failed to maintain a comprehensive water management program. During interview, the Maintenance Director stated that water temperature testing had not been conducted for the previous three months and that no water treatment activities had been completed since the start of his employment. He was unable to locate documentation of any prior water treatment program or monitoring activities, and the facility could not provide a written water management plan, hot and cold-water temperature monitoring logs, a documented water system risk assessment, a water distribution flow diagram, or documentation of corrective actions related to water system management. The Maintenance Director also stated he had not seen water temperatures, had not completed any testing, had not seen a written management plan or water system risk assessment, and did not have a distribution flow diagram of the water system. The facility also failed to provide an infection surveillance system to identify, track, trend, analyze, and respond to infectious illnesses within the facility. Review of infection control tracking and trending documentation from February 2026 through May 2026 showed unclear resident tracking, and both an LPN and the ADON stated they could not understand the information as it was sent by the DON and that it was the first time they had seen it written down. The LPN supervisor stated the DON did not share infection prevention information with her, that she had never seen a book, and that the DON said the information was on her computer. The LPN stated she was aware of residents with infections and those on antibiotics from working on the floor, but was not aware of how the DON obtained or tracked that information.
