Summary

The facility did not provide a safe, sanitary, and comfortable environment to help prevent the development and transmission of communicable diseases and infections. During wound care for R10's right forearm, RN E donned gloves, removed the old bandage, and discarded it, then cleaned the wound, applied skin prep to the surrounding area, and placed the new bandage without changing gloves or performing hand hygiene between the dirty and clean portions of the procedure. RN E later stated that gloves should have been removed after the dirty part of the procedure, hands washed, and clean gloves donned for the clean part, and the DON acknowledged that gloves and hand hygiene should have been changed between tasks. The facility also failed to follow its clean linen handling policy. The policy stated that clean linens must be transported in covered carts, but LA G was observed transporting clean linens through the building on an uncovered cart with linens exposed. The cart contained clean bed sheets, lift slings, washcloths, towels, pads, and other bed linens, and LA G stated the same uncovered cart had been used for 2 years to deliver linens to all linen closets throughout the facility. The DON and the infection control nurse both stated that the linen cart should have been covered while being moved through the facility and hallways.
