Insufficient Nursing Staffing and Missing Charge Nurse Coverage

Summary

The facility did not ensure sufficient nursing staff was provided to meet resident needs and did not consistently designate a licensed nurse to serve as charge nurse on each tour of duty. The deficiency was identified through interview and record review of the facility’s nursing staff schedules, facility assessment, and staffing policies. The facility policy stated that staffing should be sufficient to assure resident safety and maintain or attain the highest practicable physical, mental, and psychosocial well-being of each resident, and that a licensed nurse must be designated as charge nurse on each tour of duty except when waived. The facility assessment, last updated 5/30/26, documented staffing ratios for direct care staff of 1:9 to 16 on days, 1:9 to 16 on evenings, and 1:8 to 10 on nights. It also documented resident care needs, including many residents requiring assistance with dressing, bathing, transfers, eating, and toileting. Survey review of daily nursing schedules from 5/3/26 through 6/6/26 showed repeated night shift staffing below the facility’s own assessment-based requirement. Examples included nights when the census ranged from 82 to 93 residents and the schedule showed only 4 to 8 CNAs, sometimes with med techs or orientee staff counted as CNAs, while the assessment indicated 8.2 to 9.3 CNAs were needed. Survey review also showed repeated failure to designate a charge nurse on multiple shifts across the same period. On numerous dates, there was no designated charge nurse on day, evening, and/or night shifts. During interview, the Scheduler stated staffing was based on resident acuity and census, that the number of CNAs usually came from the NHA, and that some shifts did not have a charge nurse because staff were constantly changing. The NHA stated staffing was based on the number of residents and acuity on the unit, and acknowledged the facility assessment indicated a night staffing ratio of one staff to eight to ten residents while many night shifts had only six CNAs scheduled. Additional review of September 2025 staffing schedules also showed short staffing on selected dates, including nights with fewer CNAs than required by the facility assessment and shifts without full unit coverage.
