Menu Substitutions and Portion Sizes Not Followed

Pavilion At Glacier ValleySlinger, Wisconsin Survey Completed on 02-17-2026

Summary

Menus and serving sizes were not followed for lunch service, and residents were not notified when a menu item was changed. On 2/17/26, the posted lunch menu in the dining room listed red beans, rice, and sausage, a fresh baked roll, seasoned broccoli, and cheesecake. Surveyor observation showed wax beans were served instead of broccoli, and the dietary staff member serving the meal was not sure why the vegetable had changed and did not think residents were notified. The Registered Dietitian confirmed residents should be notified if there are substitutions or changes to the menu. The facility also did not serve therapeutic and mechanically altered diets as planned. Residents with pureed diets, including two residents whose orders included pureed diets and large protein portions, did not receive a dinner roll and were given applesauce instead of cheesecake; dietary staff stated dinner rolls were not pureed and that cheesecake was not pureed because of uncertainty about the crust. The Registered Dietitian stated the top of the cheesecake could have been pureed for residents on pureed diets who should receive the same items as residents on regular diets. In addition, residents on consistent carbohydrate diets were served dinner rolls even though the extended menu indicated they should not receive them. Portion sizes were also not followed: the menu listed 8 ounces of red beans, rice, and sausage, but the observed scoops provided 6 ounces for regular diets and 4 ounces for pureed and mechanical diets.
