Summary

The facility failed to provide behavioral health services to ensure a resident with a history of alcohol use, opioid abuse, anxiety, and depression received the highest practicable mental and psychosocial well-being. The resident’s care plan identified substance use concerns and included interventions such as notifying social services, offering reassurance, contacting the doctor, and providing an AA meeting calendar, but the record showed the resident continued to engage in substance-related behavior and later experienced an overdose event while in the facility.
