Summary

The facility did not establish and maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment to help prevent the development and transmission of communicable diseases and infections. The deficiency was identified as affecting 36 of 36 residents. The report cited concerns with the facility’s employee line list, which was not inclusive and did not document or track employee symptom onset, well date, last day worked, or the date an employee may return to work. Record review showed multiple employees with illness-related call-offs and return dates documented on the infection control log from March 2026 through June 2026, including vomiting, diarrhea, headache, sore throat, fever, congestion, cough, nausea, sneezing, sweats, and ear ache. During interview, the ADON/IP stated she did not track symptom onset, well dates, last day worked, or return-to-work dates, and confirmed she should be tracking those items. She also stated she would track ill employee physician notes and any influenza or COVID testing moving forward. The surveyor also observed the Nursing Scheduler delivering linens to resident rooms on the 3rd floor with an uncovered two-tier laundry cart. The cart contained towels, washcloths, chux pads, and gowns, and boxes of gloves were placed on the same cart touching the clean linen items. The facility’s policy required clean linen to be delivered on covered linen carts with covers down and stated supplies should not be on the same shelf or touching clean linens. During interview, the Nursing Scheduler stated the cart should be covered, and the ADON/IP confirmed that the cart should be covered and that supplies such as glove boxes should not be on the same shelf or touching clean linens.
