Summary

The facility did not ensure accurate and safe medication administration for 2 residents, R3 and R35, because medications were found in their rooms without physician orders for self-administration or bedside storage, without self-administration assessments, and without care plans addressing bedside medication storage or self-administration. The facility’s undated Resident Self-Administration of Medication policy stated that residents may only self-administer medications after the IDT determines which medications may be safely self-administered, that the results are to be recorded on a Medication Self Administration Assessment Form, and that bedside storage is permitted only under specified conditions with the care plan reflecting the arrangement. R3, who had diagnoses including rectal cancer, liver cancer, peritoneal cancer, and anxiety, had a BIMS score of 13 out of 15 on the 3/7/26 MDS, indicating intact cognition. On 5/11/26, the surveyor observed medication in R3’s room and R3 stated the medications were used by R3. The surveyor observed a container of lidocaine 2% zinc oxide 12% with R3’s name on the pharmacy label and a bottle of Puracyn Plus Spray without a pharmacy label, along with other wound-related products. R3’s MAR did not contain orders for bedside medication, self-administration, or for the lidocaine and Puracyn medications, and the medical record did not contain a self-administration assessment or a care plan for bedside medication or self-administration. The medications remained observed in the room on subsequent visits. R35, who had diagnoses including CHF, CKD, type 2 diabetes, and PVD, also had a BIMS score of 13 out of 15 on the 3/31/26 MDS. On 5/11/26, the surveyor observed an unopened earwax removal kit, unopened simethicone 250 mg, and unopened Soothe XP eye drops on a table at the end of R35’s bed, visible from the hallway. R35 stated the medications were needed for gas relief, cataracts, and earwax buildup. R35’s MAR contained an order for simethicone 80 mg PRN for GERD, which differed from the 250 mg tablets observed, and there were no orders for the earwax kit or eye drops. The record did not contain a self-administration assessment or a care plan for bedside medications or self-administration, and the medications remained in the room on later observations.
