Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional food safety standards, and failed to follow proper sanitation practices for the kitchen and food preparation equipment. During the initial kitchen observation, the reach-in refrigerator did not have a documented temperature for PM shift on 04/12/26 or AM shift on 04/13/26. The can opener was soiled, the box of food thickener in dry storage did not have an open or use-by date, the steamer was broken down, the inside of the microwave was soiled, the toaster crumb tray was soiled, the ice scoop was stuck inside the ice machine, one food cart was soiled inside and out, and three ceiling vents were soiled with debris. The HCSG District Manager was present and verified the findings. On follow-up, additional sanitation issues were observed in the kitchen. A soiled latex glove was on the counter beside the steamer, and a soiled cleaning cloth was on the counter beside the food processor. Two sanitizer buckets tested with sanitizer strips did not register any sanitizer, and there was no sanitizer bucket log available. One oven rack was sitting directly on the floor, one cold food container was not stored correctly, and meal trays being used to serve residents on Mary’s Garden unit were wet nesting. The microwave in the nourishment room was also soiled during observation.
