Infection Control Deficiencies in Resident Equipment and TB Screening

Cedar Ridge CenterSissonville, West Virginia Survey Completed on 06-24-2026

Summary

The facility failed to establish and maintain an infection prevention and control program related to resident ambulatory equipment and tuberculosis screening. During observation and interviews, Resident #102’s wheelchair was found to have rips and tears across the front edges of the seat exposing the inner padding, and the DON confirmed the exposed padding and removed the chair. During a facility walk-through, Resident #36’s rollator/walker seat was observed with exposed inner padding due to a tear in the right side of the plastic seat covering, and Resident #98’s geri-chair was observed with tears down both sides of the back rest exposing the inner padding. The report stated these porous surfaces could not be cleaned effectively. The facility also failed to document TB Two Step skin testing for Resident #126, Resident #37, and Resident #110 after admission. Review of the immunization tab for each resident showed no documentation of the TB Two Step skin test, and the DON confirmed that no TB Two Step PPD skin tests were administered at the time of admission per the facility TB policy. The facility policy stated that all patients will be screened for TB on admission according to CDC and local/state health department regulations and recommendations, and CDC guidance noted that skin tests should be administered to all new residents and employees as soon as residency or employment begins unless there is documentation of a previous positive reaction.
