Summary

The facility failed to coordinate care and implement wound care orders from an outside provider for Resident 1, who had a recent major gastrointestinal surgical procedure and a surgical abdominal wound requiring active care during the skilled nursing stay. The admission MDS identified the resident as having a surgical wound, and the care plan directed staff to monitor and document the wound weekly and report abnormalities to the physician. Hospital discharge instructions on 04/10/2026 directed the incision to be kept clean and dry, and the nursing home admission orders directed cleansing the midline abdominal incision, applying skin prep to the peri-wound area, and covering it with a dry dressing daily and as needed. Treatment records showed staff documented the ordered treatment until it was discontinued on 05/27/2026. After the resident’s surgical follow-up visits, the facility did not consistently obtain, review, or carry out updated wound care orders. A surgical clinic AVS directed dressing changes every 2 days with hydrofera blue and gauze, but staff stated they did not receive paperwork after the appointment and there were no progress notes or AVS in the record. A later AVS directed dressing changes every other day with hydrofera blue, yet a nursing progress note stated no new orders were received or noted. At a subsequent clinic visit, the previously healed proximal wound had opened and the distal wound was unchanged; new orders were written to repack the wounds daily with umbilical tape and cover with gauze. Staff later stated they had not previously seen the wound care orders, the wound nurse did not know about them, and the dressing had been changed without packing because supplies were not available. The resident’s representative stated the staff were not packing the wound as ordered, and the DON acknowledged the wound care was not completed as ordered.
