Failure to Provide Written Discharge Notice and Discharge Planning
Summary
The facility failed to develop a discharge plan and failed to notify a resident and/or the resident’s representative in writing of the discharge for 1 of 3 residents reviewed. Resident 1 was admitted for rehabilitation after sepsis, with a history of repeated falls, abnormal gait and mobility, altered mental status, and overall decline in functional abilities. The resident’s rehabilitation care plan stated that assistance with ADLs was needed, but the comprehensive care plan did not show discharge planning goals or a discharge plan, even though the resident’s chart later showed a discharge from the facility. Record review showed that a provider documented discharge planning issues and noted that the resident needed minimal to maximal assistance with most ADLs, could not stand without breaks, remained confused, and required 24-hour assistance. The resident’s skilled services were scheduled to end, and the resident’s representative signed a Notice of Medicare Non-Coverage stating that financial liability would begin after skilled coverage ended and that an appeal could be filed. The resident filed an appeal, lost the initial appeal, and later won the redetermination appeal, with the independent reviewer stating that the termination of skilled services was issued incorrectly and that the plan for care after leaving the SNF was not documented in the medical record. Interviews with the resident’s representative and facility staff showed that the representative was told Medicare would stop paying and that they had to quickly find an adult family home, and the representative stated the resident was not ready to leave and they felt rushed. Staff F stated that the facility did not have documents stating the purpose of discharge or notice and that notices were usually given verbally, and the resident’s EHR did not contain a completed transfer or discharge notice. The DON and Administrator stated that written discharge notices should be issued and that they inform residents of appeal rights, but the record for this resident did not show that such a notice was completed.
