Summary

The facility failed to implement documentation and monitoring of depression symptoms for one resident and failed to honor the resident’s nighttime routine preference. The resident was admitted and readmitted with diagnoses including a left leg fracture, respiratory failure, lung infection, and protein calorie malnutrition. The comprehensive evaluation dated 04/23/2026 showed the resident was cognitively intact, able to make needs known, and required assistance from one to two staff with activities of daily living. The mental health screening assessment identified moderately severe depression. The care plan, developed on 04/28/2026, noted the resident was very tearful and included a focus related to depression on admission due to the resident’s realization that they would have to sell their home and the loss of independence. The intervention directed staff to monitor and document signs and symptoms of depression, including sadness, anxiety, negative statements, and tearfulness. During observation and interview on 05/18/2026, the resident was sitting up in bed with downward turned lips and watery, glossy eyes and was on the verge of crying while speaking with the surveyor. The resident described multiple sorrowful events, including the loss of a family member and losing their home. The care plan also identified that the resident needed assistance with repositioning in bed and was on a turning schedule every two hours while awake, with a 05/07/2026 intervention stating the resident did not want to be awakened during the night shift for turning. The resident stated that a NA came into the room during the night shift and woke them to turn and change the brief, despite the resident’s preference not to be awakened. Staff interviews confirmed awareness of the resident’s sorrowful events and depressive symptoms, and the RCM stated nursing staff should have been monitoring and documenting those symptoms in the record, but documentation did not begin until 05/20/2026. The ADNS and Administrator also stated the correct process for implementing the care plan was not being followed.
