Failure to identify and manage a resident’s WV on admission

Summary

The facility failed to protect a resident from neglect when it did not identify on admission that the resident had a wound vacuum (WV) for a surgical right hip wound, did not ensure staff had the skill set and supplies needed to manage the WV, and did not implement monitoring of the WV’s operation. The resident was admitted from the hospital after hip surgery with complications, increased wound drainage, and medications ordered to prevent infection. The hospital discharge summary noted the resident was stable for discharge to a skilled nursing facility for therapy and nursing management, but the discharge paperwork did not include instructions for monitoring or maintaining the WV. On admission, the nursing assessment documented a surgical wound on the right hip with a wound bag, but the physician orders for the first two days did not identify a WV or include directions to monitor the canister, drainage, or suction function. The only admission documentation on the day of arrival was a brief nurse progress note about room placement, and the next documented assessment noted the resident was alert and oriented, required two-person assist, had right leg pain, and that the wound bag was intact. Later that day, the nurse manager documented that the WV was beeping and the canister was full; the canister was changed and suction restored, but there was no documentation of wound assessment, drainage assessment, pain assessment, provider notification, or the resident’s condition. The physician later documented that the resident required skilled nursing care to manage the WV with active drainage, wound monitoring, and medication administration to prevent infection and control pain, and directed staff to monitor WV output, dressing seal, tubing patency, suction function, and signs of infection or wound complications. Despite this, the resident’s WV was again documented as beeping, and the resident was ultimately discharged to the hospital for wound evaluation. Interviews with the collateral contact and facility leadership indicated the facility had not been informed by the central admissions team that the resident was coming with a WV, staff had not been trained on the specific WV before admission, and the facility did not have the needed supplies available when the WV alarmed and the canister filled.
