The resident’s MDS showed a BIMS score of 7, indicating severe cognitive impairment, and the resident was receiving antipsychotic and antianxiety medications. The medical record contained no evidence of a Level I PASARR screening. The Social Services Director reviewed the record and stated the Level I PASARR should have been completed at admission, with changes in medical care, or with a new diagnosis, but she was unsure who was responsible for completing it and thought it would be the DON. The Administrator stated a PASARR Level I should be completed timely and accurately.
