Summary

The facility failed to ensure that food items were stored in accordance with professional standards, and that the kitchen was kept neat and orderly with food service equipment clean or dry. During a kitchen tour with the Kitchen Manager, surveyors observed sandwiches on a plate in the walk-in refrigerator that were not dated, an opened bag of chicken cutlets in the freezer that was not dated, multiple juice spill puddles in the refrigerator, dried food particles on the standing mixer, and kitchen floors that were audibly sticky and dirty with particles on the floor. In the dry food storage room, two dented cans were found, including one being used as a door stop, and stored wet pans were stacked on each other. The Kitchen Manager confirmed several of these observations and stated that the refrigerator was cleaned at the end of each day and that the floor was cleaned twice daily after lunch and at the end of the day. On the following day, surveyors again observed the kitchen floor to be audibly sticky, the standing mixer still dirty, the pots still stacked and stored wet, the same spill puddles remaining in the refrigerator, and two more dented cans being used as a door stopper for the dry food storage room. On a later observation, the kitchen floor was still sticky and the standing mixer was still dirty. The facility's General Food Preparation and Handling Policy stated that the kitchen is kept neat and orderly, food and equipment are clean, leftovers must be dated and labeled, and all food service equipment should be cleaned, sanitized, dried, and reassembled after each use.
