Failure to Obtain Customized Wheelchair for PASRR Positive Resident
Summary
The facility failed to provide specialized habilitation services and failed to obtain specialized durable medical equipment for one resident who was PASRR positive for MI and IDD. The resident’s record reflected diagnoses including spina bifida, paraplegia, scoliosis, depressive disorder, epilepsy, polyneuropathy, reduced mobility, and chronic pain. Her care plan stated that she had an ADL self-care performance deficit related to these conditions, was at risk for pain, required pain management, and would be receiving a customized wheelchair. Her PASRR comprehensive service plan quarterly meeting also reflected that a customized manual wheelchair was ongoing. The resident’s IDT meeting in March 2026 included a request for a customized wheelchair, and the PASRR representatives stated that the initial customized wheelchair order had been placed during the 12/11/2025 IDT meeting and later placed again during the 03/05/2026 meeting. The DOR stated she attempted to request the wheelchair from the DME company, but the facility was in receivership and had outstanding bills, so the company would not continue with the process without payment. She stated the new management set up a payment plan, but by then it was already 30 days past the resident’s evaluation for the customized wheelchair and a new evaluation was needed. The DOR stated she requested an IDT for 05/26/2026 for re-evaluation, but it was cancelled due to the PASRR representative. The DOR stated the evaluation had to be within 30 days for the DME company to come and measure so Medicare could pay. The resident stated she had been waiting for her customized wheelchair for a couple of months and nothing had been done. The Administrator stated he was made aware of the wheelchair issue on 06/03/2026 and said the facility had been on hold with the DME company due to outstanding bills. He also stated the previous MDS nurse had been working on PASRR, but she quit without warning, and the new MDS nurse and social worker had just started and had no idea about the resident’s wheelchair.

Failure to Notify SMHA for New Mental Health Diagnoses

The resident’s PAS completed on 5/24/24 documented a current diagnosis of bi-polar disorder, but did not mention PTSD or personality disorder at that time. Resident #5 had diagnoses including intellectual disability and developmental disorder of scholastic skills, with a BIMS score of 9 indicating moderate cognitive impairment. His care plan identified him as PASRR positive for IDD and included habilitation and specialized services, and a PCSP meeting documented that habilitation coordination and independent living skills services were recommended. However, the EMR contained no documentation of habilitation coordination or independent living skills training notes. Interviews with the Habilitation Coordinator, MDS Coordinator, DON, Director of IDD Service, and Administrator confirmed there was no documentation after monthly visits, that the facility was not aware of when the coordinator visited, and that there was no system in place to monitor oversight or ensure the required annual and quarterly meetings were held.
PASRR Services Not Incorporated Into Resident Assessment and Care Planning

During interview, the SSD stated the Level I screening had been completed prior to admission, but the required Level II evaluation had not been completed at that time. She stated she did not recall whether the resident had been admitted under emergency admission or exempted hospital discharge, acknowledged responsibility for ensuring the PASRR process was completed, and stated the omission was identified during a chart update when the Level I PASRR screening was redone. She stated the Level II PASRR evaluation was completed later.
