The facility failed to provide pharmaceutical services to meet the needs of a resident by not ensuring that three PRN morphine orders included pain parameters. The resident was re-admitted after a short hospital stay and had diagnoses including Parkinson’s disease, dementia, and abnormalities of gait and mobility. She stated the facility’s protocol was to have an order for each dose of narcotics for monitoring purposes and described the dose she would give based on pain levels, but also stated the resident had not received morphine since readmission. The ADON and DON both reviewed the record and confirmed that the morphine orders were missing pain parameters. They stated pain parameters were important because the resident had dementia and difficulty verbalizing pain, and staff relied on facial grimacing to assess pain. Both stated it was the admitting nurse’s responsibility to ensure orders were clear and complete. The DON also stated the resident had not yet been administered morphine since readmission, and the facility’s medication orders policy required medication orders to specify dosage and be clear and complete.
