Summary

The facility failed to maintain complete and accurate medical records for 2 of 8 residents reviewed. For one resident, who was a 66-year-old female with chronic systolic heart failure, peripheral vascular disease, and non-pressure chronic ulcers of both lower legs, the record showed an order for wound care to the right and left lower legs every Tuesday and Friday. Her quarterly MDS indicated intact cognition with a BIMS score of 15 and documented venous and arterial ulcers, and the care plan addressed altered skin integrity and pain related to vascular wounds. However, the Treatment Administration Record for 06/02/2026 was blank and there was no nursing progress note for that date documenting the scheduled wound care. During interview, the wound care nurse practitioner stated she provided wound care on 06/03/2026 when she visited the facility, and said that providing the care on that date instead of 06/02/2026 was fine. The DON stated the wound care nurse did not document anything in the TAR or progress notes for 06/02/2026 and had not worked at the facility since 06/03/2026. The DON also stated she did not know whether the wound care nurse actually provided the ordered wound care on 06/02/2026 because there was no documentation in the medical record. For the second resident, an older male with diagnoses including senile degeneration of the brain, dementia, depression, anxiety, and violent behavior, the care plan identified a risk for physically abusive behaviors and directed staff to document such behaviors in the clinical record. A Provider Investigation Report showed an incident involving the resident and another resident at 10:10 a.m. on 06/08/2026, and the electronic record contained an interdisciplinary team note from 06/10/2026 discussing the resident-to-resident altercation and related interventions. However, review of progress notes from 06/08/2026 through 06/24/2026 did not show documentation of the details of the reported incident in the electronic health record, and the DON verified that the incident documentation was not present.
