Infection Control Lapses During Incontinent Care and Blood Pressure Monitoring

Summary

The facility failed to maintain an infection prevention and control program for three residents reviewed for incontinent care and infection control. Resident #45 was an older female admitted with cerebrovascular disease, cognitive communication deficit, muscle weakness, and dementia; her MDS showed a BIMS of 10 and she was dependent on ADL support. During an observation, two CNAs entered her room without performing hand hygiene and both donned disposable gloves before providing incontinent care. After removing the soiled brief, one CNA cleaned the resident’s front and back and then applied a new brief without changing contaminated gloves. The same CNA used the bed remote control with unclean gloves, and the other CNA handled the bed and blanket with soiled gloves. In interview, both CNAs stated they had contaminated their gloves by touching the resident and dirty materials and acknowledged they should have changed gloves before handling clean items. Both also stated they had received infection control and peri-care training during orientation. Resident #28 and Resident #56 were both male residents with diagnoses of hypertension. Resident #28 had a BIMS of 0 and Resident #56 had a BIMS of 9. During observation, a medication aide obtained Resident #56’s blood pressure with a wrist monitor without sanitizing it first, then used the same monitor on Resident #28 without sanitizing it between residents. The monitor was sanitized only after the second use when the omission was pointed out. The medication aide stated she had not received specific training on sanitizing medical equipment at the facility, and the DON stated staff were expected to sanitize medical equipment between residents and that additional topics related to sanitizing medical equipment had not been routinely included in in-service sessions.
