Summary

The facility failed to provide drinks, including water and other liquids, consistent with resident needs and preferences and sufficient to maintain hydration for 3 of 8 residents reviewed for hydration. During observation and interview, Resident #30 did not have a water cup available or within reach in the room, and the resident stated he did not know where the cup was and would like to have a water cup. Resident #30’s record showed diagnoses including hypertension, vascular dementia, and type 2 diabetes, with a BIMS score of 4 indicating severe cognitive impairment. His care plan stated he was able to hold a cup and feed himself and had nutritional problems or potential nutritional problems. Resident #56 was observed sitting on his bed with no water cup or water pitcher in the room. The resident stated he did not know why he did not have water and did not know that he was supposed to have water between meals. His record showed diagnoses including dementia, heart failure, and rheumatoid arthritis, with a BIMS score of 9 indicating moderate cognitive impairment. His care plan directed staff to be sure the resident’s call light was within reach and encourage him to use it. Resident #90 was observed in a room without any water, beverages, or a water pitcher at bedside. The resident stated he was not sure why he did not have water by his bedside and said he liked water. His record showed diagnoses including epilepsy, dysphagia, and Parkinson’s disease, with a BIMS score of 11 indicating moderate problems with thinking and memory. The MDS indicated he required assistance for set up or clean-up for eating and bringing food and/or liquid to the mouth. Staff interviews confirmed that CNAs were responsible for providing water, that water should be within arm’s length, and that residents should have cups filled with ice and water as needed. The DON and ADM stated staff were expected to follow hydration protocols and place water within residents’ reach, and the facility policy stated residents should receive sufficient fluid intake and be offered hydration prior to, during, and following meals.
