Record review showed that an IDT meeting was held on 2/25/26 and that NFSS needs were identified and confirmed, including specialized OT assessment and therapy, specialized speech therapy assessment and therapy, and specialized PT assessment and therapy. During interviews, the DOR stated the NFSS for Resident #4 was not submitted and that she had been unaware of her responsibility to submit the NFSS and assessments into the LTC portal for approval. She stated she was not familiar with the PASRR specialized services process and believed the LIDDA handled it. The SW stated she had never worked in LTC before and was unsure of her responsibility for arranging services decided upon through NFSS, and she needed more training on her role in obtaining and coordinating services. The Administrator stated the NFSS is completed by the DOR and should be submitted within 20 days, and that she expected the SW and DOR to attend and schedule the IDT meeting and submit the necessary paperwork within the appropriate time frame. The facility policy stated the Social Worker or designee must contact the LIDDA/LMHA within two calendar days of admission, the IDT must be convened within fourteen calendar days, and the facility must initiate the request for specialized services within twenty business days of the IDT meeting.

Resident #5 had diagnoses including intellectual disability and developmental disorder of scholastic skills, with a BIMS score of 9 indicating moderate cognitive impairment. His care plan identified him as PASRR positive for IDD and included habilitation and specialized services, and a PCSP meeting documented that habilitation coordination and independent living skills services were recommended. However, the EMR contained no documentation of habilitation coordination or independent living skills training notes. Interviews with the Habilitation Coordinator, MDS Coordinator, DON, Director of IDD Service, and Administrator confirmed there was no documentation after monthly visits, that the facility was not aware of when the coordinator visited, and that there was no system in place to monitor oversight or ensure the required annual and quarterly meetings were held.
PASRR Services Not Incorporated Into Resident Assessment and Care Planning

She stated she did not recall whether the resident had been admitted under emergency admission or exempted hospital discharge, acknowledged responsibility for ensuring the PASRR process was completed, and stated the omission was identified during a chart update when the Level I PASRR screening was redone. She stated the Level II PASRR evaluation was completed later.
