The facility failed to ensure respiratory care was provided consistent with professional standards of practice for two residents. Resident #31, a female with diagnoses including Alzheimer's disease and hypertension and a BIMS score of 04 indicating severe cognitive impairment, had an order for ipratropium-albuterol every 6 hours as needed for shortness of breath. During observation and interview, she stated she used her nebulizer face mask only as needed and was not sure when the mask or tubing had last been changed. The report states the facility did not ensure her nebulizer face mask was placed in a plastic bag, labeled, and dated. During observation, her oxygen tubing was undated or unlabeled while oxygen was running at 2 liters per minute by nasal cannula as she slept in bed. She stated she wore oxygen at all times and was unsure when the tubing and nasal cannula were changed. Staff interviews showed uncertainty about when the oxygen supplies had been changed, and the ADON stated tubing and supplies were expected to be changed weekly and labeled and dated, while the DON stated she was unaware of the policy.
