CNA Failed to Identify Herself and Wear a Name Badge

Summary

The facility failed to treat a resident with respect and dignity and to provide care in a manner that promoted her quality of life when CNA B did not identify herself and was not wearing a name badge while providing care. Resident #19 was an [AGE]-year-old female with diagnoses including senile degeneration of the brain, anxiety disorder, and essential primary hypertension. Her MDS indicated she was understood by others and understood others, and her BIMS score was 15, indicating intact cognition. She was dependent on staff for toileting hygiene and required assistance from one staff member for bathing, bed mobility, dressing, toilet use, and transfers. During an interview, Resident #19 stated that an African American CNA with brown glasses did not identify herself and did not wear a name badge, and she said she did not like this because she did not know who the CNA was. During observation and interview, CNA B was not wearing a name badge. CNA B stated she had worked at the facility for about a month, had not been provided with a name badge, and had not asked again after being told one would be provided. She said she identified herself and explained care to residents when providing care, and acknowledged that wearing a name badge was important because it was the resident's right to know who she was. The Administrator stated staff were expected to always introduce themselves and wear a name tag in a visible area, and said it was her responsibility to ensure this occurred. The ADON and DON also stated staff should identify themselves and wear name badges, and both were aware CNA B did not have a name badge. The DON said the facility was not able to get CNA B one because they did not have human resources in the building. The facility policy on Resident Rights stated residents must be treated with respect and dignity and have the right to be informed in advance of the care to be furnished and the type of caregiver who would furnish care.
