Summary
The facility failed to store, prepare, distribute, and serve food in accordance with professional standards during a kitchen observation and interview review. During the initial tour, the cook was observed not wearing a beard restraint and stated he forgot to put one on. Multiple food items were found unlabeled or undated, including a bag of turkey breast dated 05/20/26, a box of cream cheese with no date, a clear bag of onion identified by staff but unlabeled and undated, 2 chocolate pies unlabeled and undated, 4 containers of cool whip undated, a bag of mixed berries identified by staff but unlabeled and undated, 3 loaves of Texas toast bread undated, and 1 loaf of cinnamon bread undated. A bag of crumble feta cheese had a best by date of 11/18/25 and green substance was noted. The kitchen equipment and storage areas were also observed in unsanitary condition. The microwave had a brown buildup inside, the deep fryer had golden food crumbs on the inside surfaces, the dome covers were stacked with water pooled between them, and the ice machine scoop holder had several black pieces floating in the bottom. At the beverage station, water, juice, and tea were labeled 05/30/26. Staff interviews stated that all staff were responsible for labeling and dating food, cleaning the microwave, and ensuring dome covers were air dried before stacking. The dietary aide stated the beverage station items should be discarded every 24 hours, and the maintenance supervisor stated he was responsible for cleaning the ice scoop holder. Record review and staff interviews confirmed the facility expectations for food storage and sanitation. The dietician, dietary manager, administrator, and other staff stated turkey breast should have been discarded within 7 days, the crumble cheese should have been discarded by the best by date after removal from the freezer, the microwave should be cleaned daily and as needed, the deep fryer should be cleaned regularly, the dome covers should be air dried before stacking, and beard restraints should be worn in the kitchen. The facility policies and FDA guidance reviewed also required food employees to wear beard restraints, keep food labeled and dated, store opened food properly, use perishable opened foods within 7 days or less, and keep the scoop in a separate container with the handle up and washed daily.
