Summary

The facility failed to ensure that medications and biologicals were labeled and stored according to accepted standards for Resident #67, two medication carts, and the medication storage room. Resident #67 was a male with diagnoses including hemiplegia and hemiparesis following cerebral infarction and dysphagia. His quarterly MDS indicated he was rarely or never understood, had short- and long-term memory problems, and was dependent on staff for eating. His record showed orders for gabapentin 100 mg via g-tube three times daily and metoclopramide 10 mg via g-tube four times daily, and his care plan indicated he had a g-tube and was NPO. During medication administration observation, the pharmacy labels on Resident #67’s gabapentin and metoclopramide still directed oral administration rather than via g-tube. RN H administered the medications through the g-tube and stated the route had changed after the resident returned from the hospital, and that a change-of-direction sticker should have been placed on the medication cards to reflect the updated route. The RN stated the route was one of the rights of medication administration and that someone unfamiliar with the resident could have attempted to give the medications by mouth. The facility also had multiple unlabeled opened medications. On the A/B MA Medication Cart, a bottle of Active Liquid Protein was open with no date, despite the label stating it had a 3-month shelf life from opening. On the C/D Nurse Medication Cart, an insulin lispro pen, a Breztri inhaler, and two saline nasal sprays had no open dates. In the medication storage room refrigerator, an open vial of Aplisol had no open date, and a bottle of liquid Ativan was stored in a lock box that was not permanently affixed to the refrigerator and could be removed. Staff and leadership acknowledged the missing dates and the detachable lock box, and the DON stated there was no current system in place for monitoring labeling and dating of medications.
