Improper Gait Belt Use During Resident Transfer

Summary

The facility failed to ensure adequate supervision and assistance devices were used to prevent accidents for one resident during a wheelchair-to-bed transfer. Resident #4 was an elderly female admitted and readmitted to the facility with debility, immobility, wheelchair use, substantial to maximal assistance needs for transfers, non-Alzheimer's dementia, CVA, unsteadiness on feet, and lack of coordination. Her quarterly MDS showed a BIMs score of 03, indicating severely impaired cognition, and her care plan stated that transfers required assistance from one person. During an observation and interview, two CNAs were preparing to transfer the resident from her wheelchair to her bed using a gait belt. CNA I handed the gait belt to CNA J, who asked how to buckle it. The resident was asked to sit forward, and CNA J placed the gait belt around the resident's upper back directly over her breasts while the CNAs were preparing to move her. The surveyor stopped the transfer and asked how they had been trained to place the gait belt. The CNAs stated they had been trained to place it just above the hips. They then loosened and repositioned the gait belt just above the resident's hips and completed the transfer. The CNAs stated that the gait belt should be placed around the waist to prevent injury when moving a resident from a wheelchair to a bed, and they could not remember the last time they had been trained on gait belt use. The facility's in-service record referenced transfer training, but no training information was attached. The regional compliance nurse stated the facility did not have a policy and procedure on gait belt use and did not remember when staff were last trained on gait belts. A 2003 policy on moving a resident from bed to chair/chair to bed described use of a gait belt and transfer steps, including positioning the belt around the waist and using it to assist with a safe transfer.
