Care Plan Did Not Address Resident Behaviors During Direct Care
Summary
The facility failed to develop and implement a comprehensive, person-centered care plan for a resident with multiple diagnoses, including major depression, anxiety disorder, diabetes mellitus with neuropathy, non-Alzheimer's dementia, and severe cognitive impairment. The resident's annual MDS showed unclear speech, usually understood and usually understands others, with a BIMs score of 03 and no behavioral symptoms recorded. However, the care plan reviewed on 6/23/26 did not address the resident's behaviors during direct care, including resisting care, screaming when touched, pushing and hitting staff during brief changes and repositioning, attempting to bite during transfers, and cursing and insulting staff. Interviews with family and staff confirmed that these behaviors were occurring during routine care. A family member stated that CNA K had asked them to observe the resident screaming, cursing, hitting, scratching, and attempting to bite staff during care and transfers, and reported that the resident had shown similar behaviors at home when personal care was attempted. CNA K stated the resident required total assistance with ADLs, became stiff during care, and yelled loudly when touched for turning, brief changes, transfers, and personal care. CNA O and LVN Charge Nurse P also reported that the resident was confused, required total assistance, was incontinent, and would cuss, bite, hit, resist care, and scream when staff attempted to provide care. The RN MDS nurse stated she was not aware the resident had behaviors and said she did not interview nursing staff when completing the MDS assessment. She also stated she did not review CNA documentation or MARs for behaviors. The Regional Compliance Nurse stated the facility did not have a policy and procedure on completing MDS assessments and relied on the RAI Manual, and that nurses and CNAs were not documenting the resident's behaviors in the clinical record when direct care was provided. The facility's comprehensive care planning policy stated that each resident should have a person-centered care plan with measurable objectives and time frames based on the comprehensive assessment, but the resident's care plan did not reflect the documented behaviors.
