The facility failed to ensure that Resident #13 received respiratory care consistent with the facility’s policy and the resident’s care needs when the resident’s nebulizer mask was not changed weekly. Resident #13 was a [AGE]-year-old male admitted to the facility with diagnoses including respiratory failure, heart failure, and hypertension, and his quarterly MDS reflected severe cognitive impairment. His comprehensive care plan identified him as at risk for impaired gas exchange related to COPD and smoking, and his active physician order included Iprat-Albut nebulizer treatment as needed for shortness of breath. During observation, Resident #13’s nebulizer mask was found stored in a plastic bag and dated 2/16/2026. The resident was not showing signs of respiratory distress or infection at the time of observation, and he was uninterviewable. Staff interviews confirmed that the nebulizer mask and tubing were supposed to be changed weekly and as needed, with the assigned nurse expected to check that the equipment was within date and stored properly.
