Summary

The facility failed to implement a physician’s order for BiPAP for one resident who had been admitted with diagnoses including pneumonia, COPD, emphysema, obstructive sleep apnea, sepsis, and dementia. The resident’s hospital discharge summary included BiPAP settings of FiO2 102, IPAP 25, and EPAP 8, but the facility’s baseline care plan did not select BiPAP/CPAP as a special treatment, and the facility’s order summary report and MAR did not show an order for BiPAP for the resident. The resident’s wife reported that she was told at hospital discharge that the resident could use his personal CPAP machine, and she brought the machine to the facility the day before the resident arrived. The discharge orders were not changed to reflect CPAP use, and the facility did not clarify the order when the machine was brought in. The DON stated the CPAP order was not entered, so it did not appear on the physician orders or MAR, and the orders should have been changed by the discharge nurse at the hospital. During interviews, the PA stated staff did not notify him to clarify the CPAP order and that he thought the resident had a BiPAP machine. The DON later stated the facility failed to enter the CPAP order and should have clarified the BiPAP order when reconciling the discharge orders. The resident was transferred to the emergency room on 2/14/2026 at 8:30 AM.
