Unsecured disposable razors left in resident rooms and bathrooms
Summary
The facility failed to provide an environment free of hazardous materials for 7 of 99 sampled residents because disposable razors were left unsecured and unattended in resident rooms and bathrooms. The facility policy titled, Hazardous Areas, Devices and Equipment, stated that hazardous areas, devices, and equipment are to be identified and addressed to ensure resident safety, and that sharp objects accessible to vulnerable residents are environmental hazards. Resident #19, who had diabetes, depression, and Parkinson's disease and was cognitively intact with a BIMS score of 15, had 2 blue disposable razors in a cup on the bathroom counter. Resident #55, who had dementia and muscle weakness and was moderately cognitively impaired with a BIMS score of 9, had 10 blue disposable razors in a wash basin on the bathroom counter. Resident #66, who had muscle weakness, lack of coordination, diabetes, and cognitive communication deficit and was cognitively intact with a BIMS score of 13, had 1 blue disposable razor on the bathroom counter and 1 on the nightstand. Resident #73, who had chronic diastolic heart failure, anxiety, and chronic kidney disease and was cognitively intact with a BIMS score of 14, had 3 blue disposable razors on the bathroom counter. Resident #83, who had COPD, chronic systolic heart failure, anxiety, and hemiplegia and hemiparesis following cerebral infarction and was cognitively intact with a BIMS score of 15, had 3 blue disposable razors in a cup on the bathroom counter. Resident #93, who had Alzheimer's disease and dementia and was moderately cognitively impaired with a BIMS score of 8, had 2 blue disposable razors in a cup on the bathroom counter. Resident #98, who had diabetes, abnormalities of gait and mobility, and adjustment disorder with depressed mood and was moderately cognitively impaired with a BIMS score of 10, had an open package with 10 blue disposable razors in a wash basin on the bathroom counter. During interviews, the DON and an LPN stated that razors should not be left unsecured and unattended in a resident's room or bathroom, and the DON stated that staff are to obtain the razors and dispose of them after use in sharps containers.
