Summary

The facility failed to maintain dietary equipment in a sanitary manner and failed to ensure food was stored, labeled, and dated in accordance with policy. During an initial kitchen tour on 3/2/2026 at 9:45 AM, surveyors observed 1 box of 8 corn dogs left uncovered and opened in the stand-up freezer, 1 container of pimento spread with no open date in the refrigerator, 1 bowl of chicken noodle soup with a preparation date of 2/26/2026 and an invalid use-by date of 2/29/2026, 1 chicken salad sandwich with the same dates, 1 drip pan under the stove burners with a moderate amount of black burned particles and dried pasta noodles, and 1 bin each of sugar and flour that were undated. The Dietary Supervisor acknowledged the uncovered corn dogs should not have been left open, the soup and sandwich should have been discarded, the drip pan needed cleaning, and the sugar and flour should have been dated. The CDM later acknowledged that dry storage bins should have dates and that the sandwiches prepared on 2/26/2026 should have been discarded on 3/1/2026. In the nourishment room, surveyors observed multiple prepackaged freezer meals with no resident name, a disposable covered plate of unknown food that was unlabeled and undated in the refrigerator, and unknown food wrapped in plastic wrap that was also undated and unlabeled in the refrigerator door. The CDM acknowledged that all food in the nourishment refrigerator should be labeled with a resident name and use-by date, and stated the Dietary Supervisor should check the refrigerator daily. The RD also stated that foods should be dated and labeled with resident names and use-by dates, and that bins of flour and sugar should be dated. The facility policy required food-contact equipment to be cleaned and sanitized between uses and required refrigerated and frozen foods to be covered, labeled, and dated.
