MDS Assessments Not Accurately Coded for PASRR and ROM Findings

Summary

The facility failed to accurately complete MDS assessments for 2 residents reviewed for MDS coding. For Resident #13, the medical record showed diagnoses including diabetes mellitus, atrial fibrillation, and muscle weakness, and a new diagnosis of left wrist contracture was added on 11/11/2025. A PASRR Level II outcome dated 8/12/2018 showed the resident was approved for nursing facility services and met criteria for serious mental illness, including major depressive disorder and anxiety disorder. However, an annual MDS assessment and a quarterly MDS assessment both indicated the resident was cognitively intact with a BIMS score of 14, and the annual assessment marked that the resident was not currently considered by the state level II PASRR process to have serious mental illness and/or intellectual disability or a related condition. The quarterly MDS also indicated no functional limitation in range of motion of the upper extremities, even though an observation on 5/18/2026 showed the resident lying in bed with the left hand contracted. For Resident #62, the medical record showed diagnoses including PTSD, major depressive disorder, and anxiety. A PASRR Level II outcome dated 4/4/2024 showed the resident was approved for nursing facility services and met criteria for serious mental illness, including PTSD, major depressive disorder, and mood disorder. An annual MDS assessment showed a BIMS score of 15 and indicated the resident was cognitively intact, but it also marked that the resident was not currently considered by the state level II PASRR process to have serious mental illness and/or intellectual disability or a related condition. During review of the records and interview on 5/19/2026, the DON and the MDS Coordinator reviewed the assessments for Residents #13 and #62 and confirmed the facility failed to accurately code Resident #13's limited range of motion of the upper extremity and serious mental illness, and failed to accurately code Resident #62's serious mental illness.
