Summary

The facility failed to ensure an environment free of accident hazards and adequate supervision when a resident was not checked on for approximately four hours and her bed was left in an elevated position. On 4/16/26, the resident was found on the floor in her room with blood on the floor and on both hands and her right arm. She had a bleeding skin tear/laceration to her left hand, complaints of back pain, a swollen right knee, and a potential head injury, and she was transferred to the emergency room for evaluation. The resident had severe cognitive impairment, was dependent on staff for all care needs, used a wheelchair, had bilateral upper body impairment, and required substantial assistance to roll in bed. Her care plan identified that she needed substantial assistance to turn and reposition in bed and required two staff members and a mechanical lift for transfers. The record and interviews showed that staff last checked on her at about 5:00 a.m., and she was not found until around 9:00 a.m. The provider’s investigation did not explain why she was not monitored during that period or why her bed was elevated at the time of the fall. The emergency room record documented a laceration to the left dorsal hand extending across the knuckle area to the proximal phalanx, with ten sutures placed and a hand splint applied. The provider’s interviews confirmed that the bed had been higher than it should have been and that staff sometimes raised beds for care but forgot to lower them afterward. The provider’s policy required hourly safety checks with visual confirmation of the resident’s presence and safety, and the fall prevention policy required new interventions to be discussed after a fall; however, the report documented no audit related to staff completing rounds before leaving their shift.
