Summary

The facility failed to ensure two registered nurses had access to the emergency medication kit for a newly admitted resident who had been prescribed olanzapine for delirium. The resident arrived from the hospital with orders to continue olanzapine 2.5 mg in the morning and 7.5 mg at bedtime, but the bedtime dose was not delivered to the facility. A nursing note documented that the resident was a new admit awaiting medications, and the contracted RN later documented that the pharmacy had not brought the olanzapine when the resident missed the bedtime dose. During the night, the resident was restless, attempted to get out of bed, and received lorazepam. Early the next morning, staff found the resident in his room with a lighter after a small fire had ignited and damaged a plastic water mug, part of his mattress, and his bed linens. The roommate alerted staff, who extinguished the fire with water and evacuated nearby residents until the fire department cleared the area. EMS evaluated both residents, and the resident who started the fire was taken to the clinic and admitted to the hospital for observation. The contracted RN stated that olanzapine was available in the e-kit, but neither she nor another RN had access to unlock it. She called telehealth for an increased dose after the resident hallucinated and started the fire, and another nurse with access to the e-kit obtained and administered the medication. Interviews with facility leadership confirmed that not all licensed nurses had access to the e-kit at the time, and that access for all licensed nurses was not granted until after the incident. The resident’s family later reported that he had been hallucinating in the hospital as well, and the police report noted the resident appeared not to understand how the fire started and may have been hallucinating.
