CPAP Cleaning and Infection Control Failure

Summary

Provide safe and appropriate respiratory care for a resident when needed was not met when infection control practices were not followed for cleaning a CPAP machine for one resident. During observation, the resident’s CPAP machine was on the nightstand with the water chamber still attached and containing water. After the chamber was removed, a dried white film was seen on the interior and exterior sides and on the back area where the chamber connects to the machine. The CPAP housing where the chamber inserts had brown spots and dried white film on the bottom and part of the sides, and the filter at the back of the machine was dark brown. The resident stated staff were not cleaning the CPAP machine or water chamber daily or weekly and had not changed the filter in the back of the CPAP. Record review showed the resident was admitted with obstructive sleep apnea and had a BIMS score of 12, indicating moderately impaired cognition. Physician orders directed staff to clean the CPAP tubing and water reservoir with warm, mild soap weekly and allow it to dry every day shift, and to clean the CPAP weekly every evening shift. The TAR documented that staff were cleaning the CPAP machine and water chamber daily and every seven days. However, a CNA stated she did not clean the resident’s CPAP machine, believed night-shift staff were responsible, and was unaware of the filter at the back of the machine that needed cleaning. The regional nurse consultant also agreed the CPAP machine and water chamber were not cleaned and was unaware of the filter at the back of the machine. The facility’s revised CPAP and BiPAP cleaning policy required the mask/reservoir to be wiped with warm soapy water after each use and the machine to be wiped with disinfectant at least weekly, and the manufacturer’s guide directed daily emptying and wiping of the humidifier tub and weekly cleaning with mild dishwashing liquid.
