Summary

The facility failed to provide appropriate care to maintain or improve range of motion for a resident with a left-hand contracture that developed while she resided in the facility. The resident was nonverbal, had severe cognitive impairment, and was observed in bed and later in her wheelchair without a brace on her left hand during survey observations. She was noted to have a left-hand contracture, but no brace or washcloth roll was applied during one observation, and on another occasion she was again seen without the brace in place after morning care had been completed. Record review showed the resident had diagnoses including moderate intellectual disability, unspecified pain, and left-hand contracture. Her most recent therapy rehabilitation referral screening documented that she was not appropriate for therapy intervention because she had shown no change since her last therapy, and her most recent restorative program evaluation was dated more than three years earlier. The June 2026 TAR showed nursing staff documented daily cleaning and application of the resident’s left-hand brace each morning and removal at bedtime, but the 4/2/26 MDS coded her as receiving no restorative nursing brace assistance during the look-back period. This did not match the revised care plan, which stated she wore a palm protector brace on her left hand due to contracture. The resident’s current physician orders did not include active orders for the left-hand brace, including wear schedule, cleaning, tolerance, or monitoring, and there were no physical therapy, occupational therapy, or restorative therapy treatment orders related to her left-hand contracture and brace use. The care plan also did not include goals or interventions to maintain range of motion or prevent further decline, and it did not address brace use and management, therapy involvement, range-of-motion interventions, skin monitoring, positioning, or a plan to monitor effectiveness. Interviews with therapy and nursing leadership confirmed the facility had no current restorative program in place, that a new restorative aide was being trained, and that staff were unsure how long the program had been absent. Multiple staff members, including therapy and nursing personnel, were unaware of the resident’s current therapy status, brace management, or when she last received therapy services.
