Resident Eloped Through Front Entrance After Alarm Was Deactivated

Summary

The facility failed to ensure a resident at risk for elopement was kept safe and adequately supervised when he left the building through the front entrance after visitors entered. The resident had a BIMS score of 8, indicating moderately impaired cognition, and his care plan identified impaired cognitive function, the need for cueing, reorientation, supervision with ambulation, use of a gait belt and front-wheeled walker, and a potential for elopement related to exit-seeking behavior and a prior elopement incident. His records also showed repeated elopement-risk scores above the threshold for risk and multiple fall-risk scores above 10, along with a physician order for a wander guard. Camera footage showed the resident standing at the doorway when a family entered the facility, then walking out past the first set of sliding glass doors while wearing his wander guard, which activated the door alarm. Another visitor entered, and the resident then walked out through the second set of sliding glass doors. A CNA/medication aide went to the front entrance, looked out the doors, and deactivated the alarm before leaving the camera view. The facility did not initiate a code yellow when the resident eloped, and the administrator later confirmed that code yellow was not initiated at the time of the event. After leaving the facility, the resident was found lying on the sidewalk about 1.5 blocks away after a neighbor called 911. Police reported that he had abrasions to both upper and lower extremities, and the RN saw bleeding legs and arms, bruising, and the walker next to him in the ambulance. The resident was alert but confused and was transported to the ED. Hospital records later documented cellulitis of the left lower extremity, altered mental status due to hepatic encephalopathy, acute kidney injury, and multiple bruises on his torso and extremities. Interviews also showed staff uncertainty about elopement procedures and inconsistent recognition of residents at risk for elopement.
