Summary

The facility failed to ensure that residents were free from significant medication errors when a nurse did not verify a resident’s identity before preparing and attempting to administer medications. The resident involved was cognitively intact, had a diagnosis of type 2 diabetes mellitus with hyperglycemia, and did not have an order for insulin. Another resident in the facility had type 1 diabetes mellitus and had physician orders for blood glucose checks before meals and at bedtime, as well as Insulin Lispro 10 units subcutaneously before meals. According to the resident’s account, the nurse entered the room during dinner, obtained a finger-stick blood glucose reading of 192, and asked where the insulin should be injected. The resident stated that he or she did not take insulin, but the nurse said the doctor had ordered it. The nurse then presented a medication cup with two pills and explained that one was for seizures and the other was a sodium pill, which the resident said were not prescribed. The resident reported that the nurse then asked whether the resident’s name was the same as another resident living across the hall. When the resident said no, the nurse left, and the resident later reported the incident to staff, stating that insulin had been given in error. The nurse stated that she entered the room intending to medicate the roommate, believed she heard a first name matching the other resident, and then searched the EMR using that name. She acknowledged that she did not verify the resident’s identity using any required identifiers before accessing the EMR or preparing medications. She further acknowledged that the medications she prepared, including Insulin Lispro, Keppra, and a sodium tablet, were prescribed for the other resident. A unit manager later documented that the resident reported receiving 10 units of insulin in the left thigh, did not have an order for insulin, and felt lightheaded and dizzy. The resident’s blood glucose values were monitored after the event, and the physician later assessed the resident and found him or her doing well.
