Summary

The facility failed to ensure that a resident’s formulated advance directive would be followed because the paper medical record and the Electronic Medical Record (EMR) contained inconsistent code status information for Resident ID #4. The resident was re-admitted in April 2025 with diagnoses including end stage renal disease and dependence on renal dialysis. The EMR advance directive banner showed DNR/Comfort Measures Only, and a physician order also indicated DNR/CMO.
