Unsafe Transfers and Failure to Follow PT Lift Recommendation

Summary

The facility failed to keep a resident’s transfer environment free of accident hazards by not implementing the Physical Therapist’s recommendation for a mechanical lift after the resident showed a new decline in strength and walking ability, and by not ensuring staff used a gait belt during manual transfers. The resident had diagnoses including dementia with behavioral disturbance, muscle weakness, and falls, and the admission MDS indicated severe cognitive impairment and total dependence for transfers, toileting, and dressing. The resident’s Kardex indicated substantial to maximum assistance, or total dependence, for sit-to-stand transfers. A PT evaluation identified new onset muscle weakness and difficulty walking, and the PT documented that the resident required a mechanical lift for transfers until transfers were consistent for safety. The resident’s Kardex was updated to reflect the mechanical lift requirement, but the record did not show that a new Smooth Moves assessment was completed after the change in transfer status. Staff interviews showed that multiple nursing assistants and nurses were not aware of the PT recommendation and continued manual transfers with two staff members. One NA acknowledged transferring the resident manually and stated the resident could be dead weight and difficult to lift manually; another NA acknowledged lifting the resident under the arm and elbow area during a transfer. After these transfers, the resident developed swelling and extensive bruising of the left upper extremity, along with increased pain. The resident required NP evaluation, PT/INR testing, a STAT ultrasound, an x-ray, holding Eliquis for two days, tramadol for pain, and ongoing monitoring. A progress note later stated the bruise appeared to be due to improper positioning of the resident’s arm during transfers. Staff interviews also confirmed that a gait belt was not used during a manual transfer, despite the facility policy requiring gait belts for all resident transfers and ambulation.
