Summary

Appropriate catheter care and services were not provided for a resident with a foley catheter and a resident with a suprapubic catheter. One resident had a foley catheter for urinary retention and was readmitted with diagnoses including UTI due to a foley catheter and sepsis. The record showed a physician order for a urology consultation related to the new medications and foley use, but no evidence that the appointment was scheduled. The resident later had recurrent urinary symptoms, including abdominal pain, abdominal distention, hematuria, mucus in the urine, and a split genital tip with white drainage, and was hospitalized again with UTI due to a foley catheter and sepsis. The record also showed that the hospital discharge summary stated the foley should be exchanged every 6 to 8 weeks by urology and that foley exchanges should not be done at the SNF. Despite this, the facility did not arrange the ordered urology follow-up after the April physician order, after the May hospitalization for catheter-associated sepsis, or after the PA documented the need for urology referral. During interview, the scheduler stated the resident had not been scheduled until the surveyor brought it to the facility’s attention, and the urology office confirmed the facility had not contacted them until that day. A second resident with neurogenic bladder and an SP catheter was hospitalized with a complicated UTI and an obstructed SP catheter. A physician order directed the SP tube to be changed every 4 weeks, but the treatment record was signed off as completed even though staff later stated the nurses do not change SP tubes in the facility and that the resident must go to urology for exchanges. The LPN who signed the order acknowledged the entry was made in error. The urology office reported the last SP catheter exchange had been in July 2025 and that no appointment was scheduled, while the facility scheduler did not identify a urology appointment until the surveyor raised the issue.
