Summary

Provide appropriate pressure ulcer care and prevent new ulcers from developing was not met for one resident who was admitted with quadriplegia and no skin impairments. On admission and on a skin check, the resident’s skin was documented as intact, and a Braden Scale showed the resident was bed confined with very limited mobility and unable to make frequent or significant position changes independently. The resident later developed a stage 2 coccyx pressure ulcer, and a physician ordered wound cleanser, medihoney, and a foam dressing daily and as needed. The care plan and NA Kardex directed staff to avoid positioning the resident on the wound and to turn and reposition the resident at least every 2 hours with monitoring, reminding, and assistance. After the resident returned from the hospital, the facility documented another intact skin assessment, but the next day the wound nurse practitioner documented an unstageable pressure ulcer to the gluteal cleft and a stage 3 pressure ulcer to the right buttock. The wound note described the gluteal cleft wound as containing granulation tissue and slough, and new wound orders again included repositioning every 2 hours. The resident’s record did not show documentation that nursing assistants turned and repositioned the resident every 2 hours during the stay, despite the care plan and wound recommendations. During interviews, the wound NP stated she would expect the resident to be turned and repositioned every 2 hours to aid healing. The DON stated that turning and repositioning is standard practice and would be documented in the NA task section, but she could not provide evidence that the resident was repositioned as required. The Regional Director of Clinical Services acknowledged that the admission/readmission skin checks documented intact skin despite the wound NP’s findings of an unstageable wound and a stage 3 wound, and she also could not provide evidence that the resident was turned and repositioned every 2 hours. The facility’s failure to ensure the resident was repositioned in accordance with the care plan, wound recommendations, and policy resulted in facility-acquired pressure injuries, including progression of the stage 2 ulcer to an unstageable ulcer and development of a stage 3 ulcer.
