Summary

The facility failed to timely notify and accurately communicate a resident’s change in condition to the physician. The resident had multiple sclerosis, neurogenic bladder, a suprapubic catheter, and a history of UTI. The resident’s care plan directed staff to monitor for signs of UTI, catheter output, and changes in behavior. The resident was cognitively intact on the most recent MDS assessment and reported to staff that they had pain at the catheter site and believed they had a UTI. A nursing progress note by an LPN documented that the resident complained of difficulty urinating and pain at the catheter site, and that the nursing supervisor was made aware. The clinical record did not show that the physician was notified of this complaint. The resident’s spouse reported that the resident was too tired and not feeling well to talk on the phone, and later received a call from the NP stating the resident was not feeling well and labs and other orders were initiated. The spouse then found the resident lying in bed, gurgling, and not responding, and demanded that the resident be sent to the hospital. Staff interviews showed inconsistent and inaccurate communication about the resident’s condition. An RN stated they were not notified of the change in condition that morning and did not check vital signs, while an LPN stated the RN/supervisor was informed and that the vitals documented later were abnormal, including a blood pressure of 154/90 and heart rate of 111. The NP stated the office did not receive a call about the resident’s pain at the catheter site and that, if notified, IV fluids and antibiotics would have been ordered because the resident becomes septic quickly. The NP also stated the resident’s condition was presented differently than it actually was, and that immediate hospital transfer would have occurred if an accurate assessment had been provided. The resident was ultimately hospitalized with acute respiratory failure with hypoxia, septic shock, acute kidney injury, and UTI, and required intubation, mechanical ventilation, and ICU care.
