Failure to Document RN Assessments and Follow Fluid Restriction Orders
Summary
The facility failed to provide appropriate treatment and care according to orders, resident preferences, and goals for three residents. The report cited a failure to ensure RN assessment and documentation for residents who experienced changes in condition and required transfer to an acute care facility, as well as a failure to follow a physician-ordered fluid restriction for a resident with CHF. The cited standard included the RN Charge Nurse duty to complete accurate, complete, and timely assessments, and the facility policy stated that fluid restrictions were to be followed in accordance with physician orders and that water was not to be provided at bedside unless counted in the daily fluid total. For one resident with HTN, HF, and depression, the record showed a transfer to the hospital after a change in condition involving no urine output and a Foley catheter issue, but the progress notes failed to include an RN assessment for that event. The DON confirmed that the RN on call was not notified to come into the facility and that no RN assessment was completed for the change in condition. For another resident with muscle wasting, weakness, and dysphagia, the record showed an episode in which the resident was found unresponsive and gray while outside with family, with low BP, low pulse, and oxygen saturation of 68 percent, followed by 911 notification and transfer to the hospital; the progress notes did not include an RN assessment for that change in condition. For a third resident with HTN, malnutrition, and CHF, the physician ordered a no added salt diet with a 1500 ml fluid restriction. Observations showed a water pitcher at the bedside and the resident drinking from it, and staff interviews confirmed the fluid restriction had not been transferred into the TAR and was not being followed. The nurse aide and LPN both stated they were not aware of the restriction until the order was reviewed, and the PA confirmed the restriction had been ordered because of CHF and leg swelling.
