Failure to Identify and Document a Full-Thickness Toe Wound
Summary
The facility failed to identify a full-thickness right hallux wound with exposed adipose tissue in a timely manner for a resident who was admitted with cerebrovascular disease and end stage renal disease, was severely cognitively impaired, received hospice services and enteral tube feedings, and was dependent for all activities of daily living. The resident’s care plan identified risk for impaired skin integrity related to impaired mobility and included monitoring and documenting the location, size, and treatment of skin injuries and reporting abnormalities, failure to heal, or signs and symptoms of infection to the physician. Review of the wound consultant records showed the resident was seen for multiple wounds, including wounds to the sacrum, knees, tibia, ischium, right medial ankle, and right lower leg. The right hallux wound was first documented by the wound physician as a full-thickness trauma wound with infection, measuring 3.8 cm by 3.2 cm by 0.3 cm, with exposed adipose tissue and probing to bone, with signs of infection in the surrounding tissue. The wound consultant also noted that the wound required treatment orders, but the clinical record and TARs did not show documentation, assessment, or treatment orders for the right hallux wound before it was identified in the wound consultant report. The resident’s TARs showed wound care was provided for several other wounds, but there was no evidence of prescribed or received treatment for the right medial ankle, right lower leg, or right hallux wounds. The clinical record, including progress notes, nursing assessments, skin assessments, care plan, hospice notes, and dialysis notes, did not show the right hallux wound was observed or assessed before the wound consultant report. Staff interviews confirmed they were unaware of how, when, or why the wound occurred, no incident report had been completed, and the DON stated there was no documentation or communication of treatment orders for the right hallux wound before it was discovered by the wound team.
