Food Items Improperly Labeled, Dated, and Stored

Summary

The facility failed to store, label, date, and maintain food in a sanitary manner in the food and nutrition services department. Review of the facility policy titled General Food Preparation and Handling showed that food deliveries were to be placed on a flatbed cart or utility cart, and leftovers were to be covered, labeled, dated, refrigerated immediately after meal service, and discarded after three days. During the initial tour of the dietary department with the Dietary Manager, the reach-in refrigerator contained a steam table pan of gravy, a pan of cooked eggs, a pan of sausage patties, and a pan of oatmeal, and none of the pans were labeled with contents, preparation date, or discard date. Standing water was also observed on the bottom of the refrigerator. In the walk-in refrigerator, surveyors observed two pans of leftover baked ziti dated May 29, 2026, and a pan of leftover French toast dated June 6, 2026 that was covered with aluminum foil but not labeled with the contents. The French toast was stored directly beneath a refrigeration fan unit that was dripping water onto the foil covering the food, and additional water was dripping onto lower shelves and wetting food containers stored beneath the unit. Surveyors also observed a container with four raw green peppers and one onion that appeared withered and were not dated, a pan of egg salad labeled with a preparation date of June 2, 2026 and a use-by date of June 14, 2026, six cases of assorted food items stored directly on the floor in the walk-in refrigerator, one case of strawberry ice cream stored on the floor in the walk-in freezer, and 11 cases of assorted food items stored directly on the floor in the dry storage room. The Dietary Manager confirmed that food and beverages were expected to be properly labeled, dated, stored, and maintained in accordance with facility policy and food safety standards.
