The facility failed to ensure a resident received treatment and care in accordance with the resident’s wishes, the comprehensive person-centered care plan, and professional standards of practice regarding code status. Resident 1 had diagnoses including unspecified severe protein-calorie malnutrition and gastrostomy status. The clinical record showed a physician order for Full Code with a start date of May 20, 2026, and the care plan stated the resident had chosen to be a full code. However, the resident’s POLST form indicated DNR in the CPR section, creating a discrepancy between the physician order and the POLST. During interviews, the DON acknowledged the discrepancy and stated it needed clarification. Staff interviews showed that two LPNs would follow the physician order in the electronic health record if there was a conflict between the order and the POLST. The resident stated that if he became unresponsive and needed emergency life-sustaining treatment, he would want CPR. Later review of the record showed the code status had been changed to DNR and the chart flagged as DNR rather than full code, while the DON stated she changed the order to match the POLST while it was being clarified. An ADON later documented that the resident confirmed he wanted to be a full code and that the orders, POLST, and facesheet were updated to reflect Full Code/CPR.
