Summary

The facility did not ensure appropriate behavioral health management for one resident with bipolar disorder, bilateral above-the-knee amputations, and insomnia. Facility policy required nursing staff to identify, document, and inform the physician about changes in mental status and behavior, including the onset, duration, intensity, and frequency of behavioral symptoms, and to document new onset or changes in behavior regardless of risk. However, review of the resident’s nursing progress notes showed no documentation of behaviors or causes for behaviors, and there was not enough evidence that nursing staff were identifying or reporting specific behavioral changes to the physician. The resident had been accused of sexual abuse in December 2025 and again in April 2026. After the April allegation, the resident refused to change rooms or floors and was instructed not to go past room C3 on the C-Wing. During observation, the resident was seen in a wheelchair going past room C3 and became verbally aggressive when redirected, stating that staff could put him/her out if they told him/her what to do. A licensed nurse stated the resident was not easily redirected and could spit, throw things, curse at staff, and physically attack staff, but also stated that staff only redirected the resident. Psychological notes from April 2026 documented a follow-up after a recent altercation, with the resident stating he/she was not allowed to talk about the incident, while nursing staff repeatedly documented no concerns at psych visits.
