Review of resident council minutes, grievance records, facility policy, and resident interviews showed that residents repeatedly reported delayed responses to call bells, particularly on the 3:00 PM to 11:00 PM shift, but the facility did not document that residents were told whether the concerns had been resolved or that they were satisfied with the outcome. A grievance from a resident reported that a nurse aide answered the call bell, turned it off without asking what assistance was needed, and left the room. Another grievance reported that two residents waited about an hour for a nurse to respond to their call bells.
